*** Make sure you download and read the Counselor Packet - "Before you aply" ,and have your pastor and one other mail in separately to this same address their letters of recommendation.
Recommendation forms are at the end of that packet. Registration is not complete until all requirements have been met, including payment, recommendation letters, completed application, and code of conduct agreement.

After complettion of this form, please make check payable to: MCYA and write down your phone number on your check.
